I've published a new MDM version - 20120825. Hopefully the bugs which might have caused your problem have been fixed. I also heavily updated MDM internals so I might have introduced new bugs:)

I'll take a look tomorrow. I started with a fresh mds but then encountered another issue, my wife moving any drum related items to the spare room.

Ok
start MD - all looks well and 'positional In' shows snare 4 and the pads trigger
start MDM and to get the custom names to load I have to send all from both 'pads extra' and 'pads'
As seen here 'positional In' goes funny followed by a flutter of signal detection (no pads connected)

No, started from scratch and with another install of MDM.

It's a bug in the firmware. For now I can suggest you not to use Custom names for awhile. I will fix it in the next firmware release soon.
