USB Device not recognized?

Hello,
i finally got everything soldered together and plugged it in my computer. The MIDI led flashes one time and Windows XP reports that USB Device not recognized?
I have so far triple checked all solder points and found no errors there. I even tried to unplug everything except the USB wires but i got the same thing. I also tried to connect it to my Linux box
and i got the following:
[ 8246.525582] usb 2-7: new low speed USB device using ohci_hcd and address 47
[ 8246.680989] usb 2-7: device not accepting address 47, error -62
[ 8246.741382] usb 2-7: new low speed USB device using ohci_hcd and address 48
[ 8246.897563] usb 2-7: device not accepting address 48, error -62

Anyone have any hints on where to look for errors?

It could be:
1. Incorrect wiring
2. Incorrect or incorrectly programmed firmware
3. Bad cable
4. Not exactly 12MHz crystal
5. And even if you say there are no mistakes on the board, they still might be there.

You were right. The USB was wired incorrectly. Also i had to switch Anode and Cathode for the LCD backlight. I was really locked in my mind, so i took a few hours break, and tadaa the problem was solved in 15mins :D Now i have to put everything back in the box and after that reassemble my drums and start drumming!
